Your Wedding DJ Questionnaire is at the bottom of this page.

This questionnaire is due 21 days before your wedding day. Music selections and MC details cannot be changed after sending us the questionnaire.

If we do not receive your DJ /MC Questionnaire by the 21 day deadline, we will select your music for you and this music cannot be changed.

We only accept music selections and MC details that are submitted using our DJ questionnaire. Texts, emails/attachments, or streaming playlists (ex. Spotify) related to your music selections or MC details are not accepted.

Once we receive your questionnaire, your DJ will contact you to schedule your consultation.

Please remember, we must receive your Wedding DJ Questionnaire before we have your consultation or it will be rescheduled.

Your DJ will prepare your selected wedding music and email it to you for review within one week after your DJ consultation is complete.

If a second consultation is needed, you are welcome to schedule a meeting at our public office in Livonia with the owner of CC King Entertainment.
